Your first visit at Blackburn Woolfolk Dermatology involves a few steps to make your visit smooth and efficient.

Once your appointment is booked, you will receive a text message with a link to complete and sign your new patient forms, as well as office and financial policies.

We encourage you to arrive 10 minutes prior to your appointment time, so that we can ensure a smooth check-in and timely service. If you have not submitted your new patient information/forms electronically, please arrive 30 minutes early on the day of your appointment.

Policies

Late cancellation and no-show policy

We understand there are times when you must miss an appointment due to emergencies or other obligations. However, due to our high patient demand and limited availability of appointments, we have a cancellation fee.

We require a 24 hour (one business day) notice of cancellation for standard appointments, and a 48 hour (two business days) notice of cancellation for cosmetic and surgical appointments. There is a $50 fee for appointments canceled with less than 24 hours  (one business day) notice, and a $100 fee for surgical/cosmetic appointments canceled with less than 48 hours (two business days) notice.

Insurances

Before your visit, we highly recommend you contact your insurance’s customer support and confirm we are in network with your specific insurance plan. Although we have a contract with most major insurance companies, there are exclusions, and your specific plan may be out of network. 

It is also advisable to determine if your insurance plan requires a referral. Most HMO plans require a referral from your primary care physician (PCP) prior to you being seen by our office. It is your responsibility to obtain the referral from your PCP prior to booking your appointment. If a valid referral is not on file prior to your visit, you are responsible for the full cost of the visit.

We require that your insurance card be presented prior to, or at your visit. We accept temporary cards (available for you to download on most insurance’s website). A government-issued picture identification must also be presented at your visit (e.g. driver’s license, state identification, or passport).

We accept the following insurances
